Other uses and disclosures require your written authorization, which you may revoke at any time. Psychotherapy notes receive additional protection and are not disclosed without your specific authorization, except as permitted by law.
Disclosures required or permitted by law without authorization
Suspected abuse or neglect of a child, elderly person, or person with a disability; serious threat of harm to yourself or others; court orders; oversight by the licensing board; or as otherwise required by law.
Your rights
You have the right to: see and obtain a copy of your records; request corrections; request restrictions on disclosures; request confidential communications; receive a list of certain disclosures; receive a paper copy of this notice; and be notified in the event of a breach of unsecured protected health information.
Telehealth
Sessions are conducted through a HIPAA-compliant, encrypted telehealth platform. You are responsible for choosing a private location on your end.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with this practice or with the U.S. Department of Health and Human Services Office for Civil Rights (hhs.gov/ocr), without retaliation.
